Step 1: Complete registration
paperwork. Download patient forms online or
come into the office to complete them.
Step 2:
Bring your completed forms to our staff or fax them to 269-695-0412.
All forms must be completed before an appointment is scheduled.
Step 3: Make an
appointment to see a provider. We will
do our best to schedule with the provider of your choice. All new
patient appointments will be made for a time before 5:00 p.m.
